Apart from problems caused by confusion with physiologic uptake, ‘technical’ problems can also give rise to artifacts, which may be misinterpreted. g. the injection site must be scanned in the case of axillary uptake to avoid misinterpreting axillary uptake as disease when it is the result of a tissued injection. Taking a good history from the patient can help to reduce misinterpretation of scan findings. 1 Normal female scan PET/CT findings: Coronal slices are shown of a normal FDG PET half-body scan of a female.

While SUVs are usually used to quantify uptake of FDG in oncology studies, the basic principle can readily be applied to other tracers and disease. The SUV is defined as follows: SUV = Tracer uptake (MBq/mL) Administered activity (MBq)/(Patient weight (kg) × 1000) Tracer uptake is determined from a small region of interest placed over the tumor in an attenuation-corrected image, and a scanner calibration factor is required to convert the value measured from the image into MBq/mL. SUVs are based on the approximation that the tracer is distributed uniformly throughout the body, so in normal tissue SUVs are very roughly equal to one.

The uptake occurs more frequently in patients with low body mass index and in cold weather. In the cold the sympathetic nervous system stimulates brown fat to generate heat by fatty acid oxidation. This ‘nonshivering’ response to cold is more likely to occur in thin people who are more prone to feel the cold. Anaerobic glycolysis then becomes the main source of ATP production instead of lipid metabolism, and glucose turnover within brown fat is increased. This may be the mechanism for FDG uptake in brown fat.
